ProMed Staffing Resources is seeking RN Care Managers, Community Health Nurses (RNs) for new NYSDOH initiative. These are immediate opportunities, full time long-term ongoing contract positions with a premier organization! Salaries ranging from $98, 000- $ 107, 000 per year. In addition to the excellent salary we are offering medical benefits to include dental and vison, sick and vacation pay.
Job Description:
Opportunity to work in the community conducting assessments for families who have children (ages 0-21) with behavioral health/psychiatric issues. All nurses will be provided support and education for ongoing professional development and annual certification in the CANS-NY assessment tool. The assessments will be the basis for family-driven Plan of Care (POC) development to support the family' s access to Home and Community Based Services (HCBS). The Nurse Evaluator will conduct 1-2 assessments a day and work with a Family Support Coordinator to meet the family' s case management needs specifically related to the HCBS POC. This position will be field based and CANs training is offered in Manhattan office.
Coverage areas needed:
Manhattan, Brooklyn, Staten Island, Queens, Bronx and Westchester.

- Active, unencumbered New York State Registered Nursing (RN) license
- At least 2 years of field-based visiting nurse experience
- Behavioral health and or pediatric experience a++
- CANS experience a++
