Pay rate: $20 - $22 an hour depending on experience
Required Education: A BSW degree or higher.
Preferred Education: Master s degree in a human services field.
Required Experience: Minimum of one year post-educational experience working with individuals with intellectual and/or developmental disabilities.
Preferred Experience: Minimum of one year conducting assessments with individuals with intellectual and/or developmental disabilities; Experience in adapting interviewing skills to elicit information from individuals with intellectual and/or developmental disabilities, their families, caregivers and staff, and various clinicians.
Required skills: Strong analytical skills; detail and solution oriented; ability to work independently; excellent written and oral communication and presentation skills; and ability to use spreadsheet or database software; Proficient computer skills including, but not limited to, the ability use the following on a daily basis: Web-based applications, Email, Microsoft Word. Bilingual: English/Spanish
1. Schedule and coordinate the assessment process with individuals, families and providers in the home and/or community.
2. Must be willing to travel the 5 boroughs in NYC and Nassau/Suffolk counties in Long Island.
3. Contact the individual and/or family/caregiver to schedule the appointment at a time and location most convenient to that person. This will include nights and weekends at the person s request.
4. Arrange formal interpreter services as needed.
5. Identify if the individual is capable of participating in an interview (full or partial) and if there is a need for an observation. More than one visit may be required.
6. Conduct the interview/observation in person. Assessment cannot be complete without the person being interviewed or observed.
7. Utilize a person-centered interview process (e.G. Conversational style that starts with the person).
8. Interview a knowledgeable person(s) as defined by protocol who has seen the person within the assessment timeframes (e.G. Within the last 3 days).
9. Identify and interview additional knowledgeable person(s) to verify information as needed.
10. Review records available as needed for coding.
11. Code the assessment accurately based on training.
12. Utilize multiple sources (e.G. The individual, knowledgeable person, records review) to confirm and verify coding as needed.
13. Use records review to code specific items (e.G. Verification of physical)
14. Complete all assessments utilizing the provided software.
15. Ensure coding is within the prescribed timeframes of the assessments.
16. Ensure completeness and accuracy of the assessment.
17. Maintain set productivity standards
18. Meet or exceed established monthly productivity standard.
19. All assessments must meet quality standards (e.G. Assessment fully completed, individual participated in the assessment, timer frames of assessment maintained, etc.) in order to be included in expected productivity count.
